第二天早晨，罗三拐接到了尹秀贞老家派出所的协查电话，他的怀疑得到了证实，瞿宗平和尹秀贞曾经是一对秘密情侣。他和关大铭把袁润等请到医院现场，揭开了杨翠花和柳玉娇被害的真相。原来瞿宗平对人民政府接管医院非常不满，对自己失去的权利利益耿耿于怀，与军代表的矛盾十分尖锐，更对柳玉娇投身徐春磊痛恨不已，遂精心策划并利用尹秀贞实施了这两起谋杀。然而，这只是他的推理，缺少足够的证据，他要求立刻逮捕尹秀贞。正在这时，田林赶来报告，尹秀贞死在自己寝室里了。尹秀贞躺在床上已经死亡，手上握着一支注射过“异丙肾上腺素”的针管。寝室门窗紧闩，外人无法进入，凶手也显然不能在作案后离去时从外面闩上门窗。而且她死得很从容很平静，还留下了遗书，现场所有迹象都表明，她是自杀。尹秀贞早不自杀晚不自杀，刚刚选在这时自杀定有蹊跷，完全可能是瞿宗平杀人灭口。但检定表明她的死亡时间为夜里一点到三点之间，瞿宗平此时正与田林等人远在江口镇出急诊。不管怎么说，只有解开“密室”之谜，即找到在这种“密室”条件下存在凶手作案的可能性，其他的推测和调查才站得住脚，所以这是全案的关键和首要。但尹秀贞的房间没有破绽。鉴定报告出来了，遗书确是死者所写。另外，死者生前患有较严重的肝腹水，有过性史并做过人流刮宫手术，时间大约在两年前。关大铭心里一动。田林根本不相信尹秀贞堕过胎，说她信教，生活严谨守身如玉。与此同时罗三拐查了三年来医院的考勤纪录，尹秀贞只在前年春节回过老家，而那段时间瞿宗平也恰好不在医院，很可能这次手术是在老家私下做的。根据掌握的情况，关大铭大胆推测尹秀贞的确是自杀的，但她更是被瞿宗平吓死的。瞿宗平利用她对自己的崇拜与感情，故意把她的肝病往肝癌上引导，使她感到绝望而自杀。关大铭决定传讯瞿宗平正面交锋，希望能打开缺口，抓住他的破绽。但是传讯毫无收获，瞿宗平表现得非常从容滴水不漏。他很明白尹秀贞一死公安就无法查证自己的言行了。当事人均已死亡，老谋深算的瞿宗平真的就逃脱惩罚了吗？
