叶常绿(黄德斌饰)为朝廷刽子手，需在秋分前赴京上任，却在路途上遇到一列流放边疆的队伍，其中一名脱逃孕妇阮素心(姚子羚饰)赫然是恩公吕大人的小妾。数月前，吕大人因冒死进谏而被斩，而执行斩首之责的正是常绿﹗常绿本已为此极之内疚，如今又见素心怀有吕大人的骨肉，腹大便便，因而誓要保护素心以赎其过。 因缘际遇下，常绿带著素心寄居在京城最出名的稳婆花蕊红(邵美琪饰)家里。蕊红察觉常绿为人冷酷，行径古怪，一度怀疑他欲谋财害命。正当蕊红感到危机重重之际，常绿先发制人迫令蕊红替自己保守关于素心的秘密，蕊红碍于眼前形势无奈答允。 某夜，常绿见蕊红迟迟未归，怕蕊红因此事受牵连，于是带著鬼头刀上山寻找。常绿找到蕊红之馀，还意外发现不知名孕妇被埋尸山上。该女子虽已气绝，惟蕊红判定胎儿仍有活命机会，遂以素心的秘密胁逼常绿劏肚救胎。常绿只好挥下鬼头刀，剖腹取子。
